We see patients by appointment in our office Monday through Friday. Office visits are scheduled as quickly as possible and every attempt is made to accommodate your needs. We appreciate 24 hour notice should you need to reschedule your appointment.


We participate in most major insurance plans, and Medicare. Please contact our billing office at 850-656-2006 with any specific questions about your insurance coverage or co-pay requirements.


On your first visit, we will begin a permanent record. You will complete a registration form at this visit. Information required will include past medical and surgical histories, allergies to medications, and a complete list of your current medications with dosages. Please bring your insurance card with you, and a photo ID.  If there is a co-pay with insurance, we will collect it at check-in.


Prescription refills, including pain medicine, will be handled Monday through Friday 9:00 am to 4:00 pm when the Doctor is in the office and has access to your medical records. Refills should be requested before your supplies are exhausted.


Your medical insurance is a contract between you and your insurance company. However, as a courtesy to you, our office will bill your insurance company for your medical expenses. If you are not covered by a carrier we contract with, payment must be made in full at the time of service. If you do not have medical insurance, payment must be made in full at the time of treatment unless other arrangements have been made in advance. For your convenience, payments for medical expenses may be made in cash, by check or credit card.


We assure you that your medical records will be handled with the strictest confidentiality. We will not give out information regarding your condition to your employer, friends, relatives, or insurance company without your written permission.
